Getting Ready for Your Visit

At DTW, we are dedicated to ensuring that your experience with us is relaxed and stress-free. To better assist our patients, we’ve added HIPAA Compliant forms you can access and submit right from our website.  Please complete and submit the forms prior to your visit.

You will need to bring the following items to your visit: 

  • Photo ID
  • Insurance information and card
  • A list of your current medications
  • Any and all x-rays films if applicable
  • Information about what treatments and medications have been tried in the past
  • Medical records from prior visits that you think may be helpful

There are two ways to complete the required paperwork for your visit. The first is our HIPAA Compliant  Online forms – you can access and submit right from our website, all without printing. The second way is to fill them out by hand and bring to your visit.  Please see below for access to the two different options.

Billing & Insurance

We participate with most local and many national insurance plans.

  • And most major insurance carriers

However, it is your responsibility to understand whether your insurance has limits on the doctors you can see, or the services you can receive.

If you provide complete and accurate information about your insurance, we will submit claims to your insurance carrier and receive payments for services. Depending on your insurance coverage, you may be responsible for co-payments, co-insurance, or other deductible amounts.
